New Hampshire Living Will Form | Advance Directive

The New Hampshire living will form requires at least one notary or two witnesses and applies according to the US statutes §§137-J:1 to 137-J:16. It facilitates a patient`s wishes regarding their medical treatment they will receive if they are so ill that they are no longer or able or rational enough to make these choices. These choices include any specific individual preferences the principal may have.

The principal must indicate if they wish to receive or withhold artificial hydration and nutrition by initialing either (a) or (b).
