The Arizona living will form is also known as an Advance Health Care Directive. This document is created with respects to the US statutes §§36-3201 to 36-3297 and allows a person, who is usually referred to as the `Principal`, to legally choose the medical treatment they will receive should they become incapacitated to such a degree that they are no longer or able or rational enough to make these choices. An Arizona living will form requires at least one notary and one witness.

Arizona durable medical power of attorney form is a document which legally allows a patient to appoint another person (usually called an `Agent`) to make decisions regarding a patient`s health care treatment should they become incapacitated to a level where they are no longer able to make these decisions themselves. The document requires at least one notary and one witness.  If their is only one witness then they may not be in any way an agent, provider or relative of the principal. The document is in accordance with the US statute §36-3201.
