Free Maine Living Will Forms | Advance Health Care Directive

The Maine Advanced Directive, is drawn up in accordance with Statutes tit. 18- A, § 5-801 to -817 giving  individuals, referred to as a `Principal`, the ability to express their medical health end of life and  decisions should they at any point become unable to through serious illness. The form must be completed in writing requires two witnesses. The principal will not be able to use this document if they are found to be pregnant.

The Maine medical power of attorney form is created accordance with the US Statutes tit. 18- A, § 5-801 to -817 for the principal to have the right to an appointed authority, the agent`, in the event of them being incapacitated through illness, illnesses such as a coma. This authority also includes the decisions regarding end of life treatment. The form will not be a legal valid document if the principal is at any time pregnant.
