The Oklahoma Advance Health Care Directive form, or Living Will is created with respects to Statutes §§3101.1 to 3102A to allow a person to legally dictate to medical staff the sort of care they will receive in the event they are no longer able. This is at such a time that illness has caused them to be unable to communicate. The document allows them to express things like whether to receive life sustaining treatment or the specific treatment they wish to receive.
How to Write
Advanced Directive For Health Care
Part I: Living Will
1) In the event of impending death the principal must initial the relevant fields to declare their wishes regarding life sustaining treatment.
- To only receive artificial hydration and nutrition.
- To receive any life sustaining treatment.
- To receive all life sustaining treatment including artificial hydration and nutrition.
- To refer to more specific instructions in paragraph 4.
2) In the event of becoming permanently unconscious or unable to communicate through illness the principal must initial the relevant fields to declare their wishes regarding life sustaining treatment.
- To only receive artificial hydration and nutrition.
- To receive any life sustaining treatment.
- To receive all life sustaining treatment including artificial hydration and nutrition.
- To refer to more specific instructions in paragraph 4.
3) In the event of an end of stage condition the principal must initial the relevant fields to declare their wishes regarding life sustaining treatment.
- To only receive artificial hydration and nutrition.
- To receive any life sustaining treatment.
- To receive all life sustaining treatment including artificial hydration and nutrition.
- To refer to more specific instructions in paragraph 4.
Other Wishes Or Conditions
4) The principal must enter the details of any other specific conditions or limitations regarding the medical health care treatment.
Part II: Appointment Of Health Care Proxy
The name of an agent and an alternative agent must be entered.
Part III: Anatomical Gifts
The principal must initial the relevant fields to state their wishes regarding donation of an anatomical gift, it`s purpose and the specific body parts to be donated.
Part IV: General Provisions
The principal must enter their personal details to acknowledge their understanding and acceptance of this document.
