South Carolina Living Will Form (Advance Directive)

The South Carolina Living Will Directive has been designed to give a patient the ability to still make choices over the health care they will get if they are somehow incapacitated and unable to do so. The patient can use the document to pass on very specific details as regards how they wish to be treated, from their religious and spiritual preferences to ending of life decisions . This will conforms with US statutes §§44-77-10 to 44-77-160 and  requires two witnesses and a notary.

If in a persistent vegetative state or permanently unconscious.
