The Wisconsin Advanced Directive,  This allows  individuals, often referred to as a Principal, to express their end of life and medical health decisions in case they are at any point they are unable to. The Wisconsin advanced directive is drawn up in line with the US Statutes  §§ 154.01 to 154.29. The form requires two witnesses. It is not legally possible to use this directive  if at the time they are found to be pregnant.

The Wisconsin medical power of attorney document, is created accordance with the US Statutes §§155.01 to .80 and gives the patient the right to an appointed alternative authority, the Agent`should they be suffering from an illness which causes them to become incapacitated. This authority also includes the decisions regarding end of life treatment for example making an anatomical donation. The power of attorney requires a notary and two witnesses for it to be legally binding.
